Once you have reached out to the Human Resources Director for availability, you may schedule an interview. When you arrive for your interview be prepared to completely fill out an Application of Employment. The Human Resource Director will supply you with the application and take you back for an interview once you have completed it.

Please be aware that all successful applicants must be 21 years old. If you are offered a position after completing the interview, you are required to turn in the following documents before you will be allowed to start working:

  • Copy of your High school diploma or GED Certificate
  • Copy of your Drivers License
  • Copy of a current Fingerprint Clearance Card or Receipt from Active Application
  • Copy of Certified Personal Caregiver certificate (if applicable)
  • Copy of a current CPR/First Aid Card
  • Copy of 2 current TB Test (dated no more than one year apart) both with Negative results
  • Copy of a current Physical (dated within the last 6 months)
  • Copy of a 39 month motor vehicle record

The Human Resources Director can assist you with obtaining any of these and be sure to ask about Sign On Bonuses, Referral Bonuses, and Raises for obtaining a Certified Personal Caregiver certificate.
