We are currently contracted with all Regional Behavioral Health Authorities (Mercy Care &amp; AZ Complete Health) to provide services to Title and Non-Title XIX adults designated SMI, as well as adults who utilize ALTCS and are appropriate to receive Personal Care services in a BHRF setting. All of our facilities have additional Personal Care licenses to ensure we can help folks stay in their communities rather than be prematurely hospitalized\/institutionalized. We also have contracts in place with United Healthcare (Optum) and BCBS (Health Choice) to provide services to adults who utilize ALTCS.<\/p>\r\n\r\n\r\n\r\n<p>Eligibility is determined after a member of our clinical team reviews the referral documents and schedules a screening with the individual being referred (and with the guardian\/advocate if applicable). If all parties involved mutually agree to scheduling an initial screening, a member of the Alliance Behavioral Care clinical team will conduct an initial screening and assess the individual&#8217;s willingness and clinical appropriateness for treatment.<\/p>\r\n\r\n\r\n\r\n<p>If an individual is accepted into our program the following items are required upon admission or prior:<\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Face Sheet with correct contact information- PCP, Payee, Advocate, Parents, Guardian, etc.<\/li>\r\n<li>AHCCCS Eligibility Verification- copy of card or printout<\/li>\r\n<li>Physical\/Nursing Assessment- must be dated within seven days of admission<\/li>\r\n<li>TB Test- conducted within the last six months with negative results<\/li>\r\n<li>Part D\/ISP- Must have current signatures and be less than six months old<\/li>\r\n<li>Part E\/Comprehensive Assessment- Must have current signatures and be less than six months old<\/li>\r\n<li>Current Medications\/Doctors Orders\/Written Prescriptions<\/li>\r\n<li>Current At Risk Crises Plan- if applicable<\/li>\r\n<li>Copy of Court Ordered Treatment- if applicable<\/li>\r\n<li>Copy of Guardianship Documentation- if applicable<\/li>\r\n<li>Copy of Probation and Parole Documentation- if applicable<\/li>\r\n<li>Copy of Conditional Release Documentation- if applicable<\/li>\r\n<li>Copy of Payee Documentation and Contact Information- if applicable<\/li>\r\n<\/ul>\r\n","protected":false},"excerpt":{"rendered":"<p>Alliance Behavioral Care provides services to individuals from a wide variety of backgrounds and geographical locations.
